Article talks about only a single city, Ekaterinburg, which has nearly 2% of the population infected with HIV. (Upward of 1% is considered an epidemic.)

However, systematic testing is not practiced countrywide. This epidemic in Ekaterinburg has only been announced because of a renewed focus on testing citywide. If and when testing proliferates in other cities and regions, the statistics will rise.

lack of testing
lack of awareness of the risks (inaccurate record keeping, issue has been played down / ignored at government level)
low funding and low effectiveness of existing programs
unprotected sex (homo and hetero)
drug usage
unsterilized medical supplies

But this did not happen yesterday, she said, while noting that the epidemic has not been announced officially.

The Sverdlovsk region where Ekaterinburg is located tops the list of Russian regions afflicted by HIV. In the first nine months of 2016, the HIV infection rate there stood at 118.2 per 100,000 people.

On the other hand, the Sverdlovsk region also has more people tested for HIV – over 23 percent of the population – while in other regions only 15 people are generally tested, according to Savinova.

The infection is ‘getting older,’ with many people infected now 30-49 years old – and not all are from society’s marginal layer.

“These are not drug addicts or homeless people; they are socially prosperous people. A mass infection took place in the 1990s, when prostitution and drug addiction were wide spread,” Savinova said.

“Ekaterinburg’s health authorities have registered some 26,693 cases of HIV,” the city’s mayor, Evgeny Roizman, said, as cited by RIA Novosti. That amounts 1,826 cases per 100,000 people, or 1.8 percent of the city’s population.

Earlier, in an interview with Ura.ru news portal, Savinova said that the majority of those infected receive antiretroviral drugs, but this is still not enough, and the HIV epidemic is spreading.

“If [the Russian authorities] don’t take action in the next five years, the situation could become absolutely deplorable,” Savinova said.

Savinova drew an example from UNAIDS 90×90×90 strategy aimed at battling HIV and AIDS worldwide, whose targets stipulate that, by 2020, 90 percent of all people living with HIV “will know their HIV status,” 90 percent of all people diagnosed with HIV “will receive sustained antiretroviral therapy,” and 90 percent of all people “receiving antiretroviral therapy will have viral suppression.”

On Wednesday, Russian Prime Minister Dmitry Medvedev ordered that 2.28 billion rubles ($36 million) be provided for treatment of those afflicted by HIV and Hepatitis B and C.

In 2015, after the Russian Health Ministry published a report cautioning that the HIV epidemic in Russia was likely to spiral out of control by 2020, Medvedev urged it to develop a plan to combat HIV and AIDS.

In July of this year, the United Nations AIDS agency (UNAIDS) said that there are some 36.7 million people infected with HIV worldwide, and that number has continued to grow over the past five years.

The agency called for “urgent and immediate” action to prevent the further spread of HIV, as a stream of new cases has brought a steady decline in the number infections to an end. Around two million people now contract the virus on an annual basis, globally.

Very interesting. I always assumed Russia is pretty tough against these sort of thing as , in order for a non-Russian to get a work visa in Russia and relocated there, you must conduct an HIV test and of course score negative, or else you won't get the work permit.

I had to get an AIDS test to every time I needed a new nine-month work visa (yes, a new test for each visa).

I didn't have to get tested at all for a three-year tourist visa. Imagine that.

It was sometime ago that I watched this documentary and I can't recall what the documentary was called but there is a few cities in Russia that have a major heroin problem. So bad that the poorer addicts are using an alternative called Krokodil (google it, nasty stuff). So an aids epidemic is more than likely due to a sharing needles for heroin/krokodil use.

Articles mentions the use of the drug is more commonly used in Russia. The drug is also 10 times cheaper than heroin. 1/3rd of heroin related deaths in the world occur in Russia. Also a drug use by opticians to dilate pupils are commonly being injected. The drug is popular (if that is the correct word) in Siberia. Which just happens to be where Yekanterinburg is.

Russia's bigger problem is that the criminal Yeltsin thwarted Gorbachev's reform process when he was (though the ultimate responsible rests with the 8 hardline traitors who arrested Gorbachev in the first place) dealing with a coup, and then let all of the USSR's industries and resources fall into the hands of more criminals. It is true that Russia today is aggressive to its neighbors, and it is true that Putin is corrupt—but while this corruption was being built up the West and their newfound Eastern European toadies stood by and watched with no small amount of schadenfreude as the Cold War rival state destroyed its own sovereignty, society, and security. The more I read about this the more I think the Russians are perfectly rational to distrust the West and break the "civilized" Westphalian norms.

Hookers tend to take better care of themselves since they know the risks. Closet bi's and former inmates are likely a big vector. Women are at risk of catching HIV from raw dog. Another unsuspecting man having unprotected anal with said woman is at risk of catching it from her and then passing it on.

F2M transmission is rare but it can and does happen.

Yes, it is possible to explain away those who lead a dangerous lifestyle knowingly as degenerates.

But as a matter of public health policy, they need to be isolated and treated to prevent those who accidentally come in contact with them from catching the disease.

Your biggest risk factor will be medical care - you'll want to buy your own consumables (syringes and needles). The economic stagnation has hit budgets hard, and medical care wasn't really free even before gringo tax comes into play.

It is heroin abuse pure and simple, remember how close Afghanistan is. Add heroin and Brazilian levels of anal sex and you get a high HIV rate. I know a lot of Eastern European people and they all tell me the same thing-Alcoholism, heroin abuse and lots of prostitution are a problem-also hopelessness, but that usually goes hand and hand with heroin abuse. I did not say Brazil's HIV rate was high, but the popularity of anal sex there is pretty high. All things ass is pretty popular in Brazil, and anal sex is hugely popular in Russia. There was a story about teenage prostitutes from Russia in Vice back in 2006 and the whores were complaining that guys only want to fuck them in the ass once they get over 18. The whores were also heavy into heroin.

There have been a lot of changes since the 1990s though. People over there are a lot more established now even during the recent crisis. It didn't cause as much friction as what was expected. You will also find that during the 00s, Russian culture became more conservative while incorporating internationalism when it suited them. I.e. Sushi, Western office style work place, travel etc. However taking a hard line stance against drinking and vice, as most families probably have some horror stories about the dangers of alcoholism.

Sociologists and academics into IR viewed Russian society as if it was collapsing. Perhaps that was the case in the late 90s but in the last ten years or so the reverse seems to be happening. Russians may still think this but they are cynical and have rose tinted glasses about how other countries operate. Although this is starting to change now as kids coming back from the U.K and France, U.S to live in Moscow and St Petersburg; are making comments on how dangerous places like Paris have become in comparison to back home.

Corruption is still an issue although it is more covert now and is starting to rescind away from the bottom. Police officers are less likely to take bribes now and seem to becoming better. However it's still a risky country for investors, to do well over there you do need to be a bandit by spirit of some kind it seems.

Problem is with debating the issue of Crimea and making statements about Russia defying Westphalian norms. Is that the coin can always be flipped, I remember hearing a lot of debates between ethnic Russians from the Baltic states and English students. "Why does Russia always sabre rattle the West", "Well why does the West always sabre rattle Russia".

I didn't say Russia was collapsing, but that the West pretty much watched with glee as it suffered extreme economic and social chaos in the 1990s. Granted the West didn't have a responsibility to help but there was a lot that could have been done to not rub them the wrong way.

Corruption is more of a systemic problem, like in China. Just because the average person might not sense it doesn't mean it isn't there and endangering the nation.

Russia's health minister announced that there are 10 Russian states with "critical" levels of AIDS, the majority of which are related to injectable drugs, particularly heroin. 57% of AIDS vectors relate to injectable drugs.

Highest existing levels are in Irkutsk, with 1538 existing cases per 100k. Highest infection rate is in Kemerovo, with 248 new cases per 100k.

The big difference between US and Russia is the level, accessibility, and affordability of care, combined with the rates of growth. Unfortunately, AIDS prevalence there will keep on growing.
